Key Request Instructions

Procedures for Issuing Keys

(Check Out and Check In)

To check out a key:

At least 5 business days before the employee needs the key(s), the head of the departmental library or unit, (or their delegate) needs to submit a request for the person needing key(s).  One form needs to be completed for each employee.  Please do not list multiple people on one form.  The request form link is:

Library Facilities will complete a Key Authorization Form.

  1. Library Facilities will retrieve a key already in stock (can take up to 48 hours), or if a key needs to be ordered from the key shop, the request may take up to 14 days to receive key(s).
  2. Library Facilities will schedule key pick-up via Outlook calendar.
  3. The employee must come to Room 44 to retrieve their key(s).

To return a key:

  1. The unit head or supervisor needs to send the employee to Room 44 to hand in their key(s). Key return open office hours are Tuesdays 8:00 am – 9:00 am, Wednesdays 12:30 pm – 1:00 pm or by appointment.
  2. The process is done!

Lost Key(s)

There is a $25.00 fee for each key that is lost.  Checks need to be made payable to the University Library and turned into someone in the Library Business Office.  Cash, credit, and debit will not be accepted.
