These forms create tickets in the TeamDynamix IT and Facilities queues.<\/p>\n<p><span style=\"text-decoration: underline\"><strong>1) Workspace Readiness Form:<\/strong><\/span>\u00a0Supervisors, please fill this out as soon as you have approval to fill or search a position so that IT &amp; Facilities have as much lead time as possible in case repairs or upgrades are needed to the workspace. The form is located here: <a href=\"https:\/\/go.library.illinois.edu\/workspace_readiness\"><span style=\"text-decoration: underline\">go.library.illinois.edu\/workspace_readiness<\/span><\/a><\/p>\n<p><span style=\"text-decoration: underline\"><strong>2) New Employee Starting Form:<\/strong><\/span>\u00a0Supervisors, please fill this out as soon as your candidate accepts the employment offer (this is also included as an item on the checklists below). It collects specific information about the new hire\u2019s name, start date, etc. The form is located here: <a href=\"https:\/\/go.library.illinois.edu\/new_employee\"><span style=\"text-decoration: underline\">go.library.illinois.edu\/new_employee<\/span><\/a><\/p>\n<p><span style=\"text-decoration: underline\"><strong>3) New Employee Onboarding Checklists:<\/strong><\/span> Supervisors, AFTER you have submitted the 2nd form, it is usually the time to begin using the <span style=\"text-decoration: underline\"><a href=\"https:\/\/library.illinois.edu\/bhrsc\/new-employee-onboarding-checklists\/\">New Employee Onboarding Checklists<\/a><\/span>.
