Re: Personal Annual Reports - 2004 Reporting Year
The enclosed packet contains all of the information you will need for writing and submitting your 2004 personal annual report. ALL FACULTY (including visiting and half-time) and Research Associates are required to submit a personal annual report and a current curriculum vita. This also includes faculty who may have been on sabbatical for all or part of the year.
The annual report should not include documentation, letters, or other attachments. The font used on your report should be 11 point or larger. The librarianship section of the report must not exceed 3 pages. This limit is strictly enforced. One point will be deducted from your librarianship score if the librarianship section of your annual report exceeds three pages.
Annual Reports and accompanying curriculum vitae will be due in the University Librarian’s Office on February 14, 2005. Reports submitted after the deadline will not be included in the review or ranking process.

February 14, 2005 – Each faculty member submits to the Office of the University Librarian their personal annual report. Please provide 10 copies of your annual report (1 for the University Librarian's office, and one for each member of FRC). In addition you must provide a current, updated curriculum vitae. (Please bring one copy of your vitae to the University Librarian's Office. (The Human Resources Office would also appreciate receiving an updated vitae.)
April 1, 2005 – Peer Review Committee reports are due to FRC for tenure-track faculty.
April 1 – May 2, 2005 – The Faculty Review Committee reviews the documents.
May 2, 2004 – The Faculty Review Committee forwards scores and rankings to the Office of the University Librarian.
Each evaluated area (Librarianship, Research, and Service) will be given a specific weight as follows: Librarianship 50%, research and creative works 30%, and service 20%. The included FRC Scoring guidelines explains the criteria for evaluating and scoring each area.
